Centrus Premier Homecare, Inc. understands that no two patients are the same. That’s why our homecare services are tailored to meet the needs of each person that we serve. Our Clinical Case Managers will work with you and your family, Physician, medical facility staff, and insurance providers before, during, and after discharge to make the transition from a medical facility to home as smooth as possible.

Our Process

We will send a Registered Nurse to either your home or discharging facility within 24-72 hours at no cost to you to assess the level of care needed for you or your loved one.

Client/Caregiver Match

We work hard to match the patient’s needs to our experienced and compassionate team of healthcare professionals to ensure comfort of care.

Visiting the Patient

Centrus will meet one-on-one with our patient and his/her family to discuss and address any concerns.

Home and Safety Assessment

A Clinical Case Manager will perform a complete home and safety evaluation, and order any medical equipment and pharmaceuticals that are needed by the patient.

Continuous Supervision

The Centrus team will continually supervise and evaluate each case to ensure complete satisfaction.
